Diabetes Health Directory Project
To meet the needs of the Royal Hobart Hospital (RHH) Diabetes Health Directory project the TAS eHealth Directory (the Directory) has been expanded to include public and private diabetes related service providers in the south of Tasmania. This enables general practitioners, diabetes health professionals and other health providers to easily access comprehensive and up to date information about diabetes service providers and services available. More specifically the Directory now;

Includes diabetes service providers consisting of diabetes specialists, diabetes nurse educators, dieticians, psychologists, podiatrists and exercise physiologists. It also includes diabetes organisations (eg Diabetes Tasmania, Heart Foundation), as well as local and state government initiatives (eg chronic disease self management activity programs) in southern Tasmania.

Provides up to date information on provider details, services and locations, to inform health professionals working in the area of diabetes about actual and potential colleagues and available services promoting better communication.

Provides a web based system available to all health providers and consumers for easy access and navigation for those seeking information and referral to diabetes services that is easy to navigate and access, with links from other sites eg, Department of Health and Human Services (DHHS), TAS eHealth and Diabetes Tasmania etc.
Examples of Functionality
Below are some examples of how the directory is able to be used to benefit health professionals looking for and providing diabetes services.
Case Study 1: A GP may be setting up a GP Management Plan for a patient with Diabetes. Through the Directory the GP is able to search for any health professionals to participate in this program. The patient may be required to visit a podiatrist. If the GP searches the Directory for GPMP, Podiatry Services participating in this service will be displayed.
Case Study 2: A consumer or health professional isable to search for providers participating in government (local or national) diabetes initiatives, e.g. Diabetes Assist COACH program or the Lifestyle Modification Program.
